摘要

背景:阿尔茨海默病(AD)和行为变异额颞叶痴呆(bvFTD)是痴呆症的重要病因,在许多病例中具有挑战性的鉴别诊断。阿登布鲁克的认知检查-修订(ACE-R)是一种认知电池,可能有助于区分这两种疾病。目的:探讨ACE-R结合社会人口学因素在AD与bvFTD鉴别诊断中的价值。 方法:ACE-R应用于102例可能由AD引起的轻度痴呆患者,37例轻度bvFTD患者和135例对照患者。通过logistic回归和ROC曲线分析患者和对照组的表现,以提高ACE-R对AD和bvFTD的诊断准确性。 结果:ACE-R的注意和定向、流畅性和记忆评分结合学校教育将AD与对照区分开来,ROC曲线下面积(AUC)为0.936(86%敏感性和87%特异性)。ACE-R的注意和定向、流畅性和语言评分,结合性别(男性)、年龄和学校教育,将bvFTD与对照组区分开来,AUC为0.908(81%的敏感性和95%的特异性)。在区分AD和bvFTD时,ACE-R评分中注意和定向、流畅性和语言以及年龄的AUC为0.865(78%的敏感性和85%的特异性)。 结论:ACE-R评分与社会人口学数据的结合可以很好地区分研究样本中的AD和bvFTD。
