阿片类药物危机时代术后恢复方案实施对术后疼痛管理的影响:化疗后开放性RPLND经验

摘要

目的:增强术后恢复(ERAS)方案已被证明可以优化术后结果;然而,由于相关的副作用和潜在的并发症,阿片类止痛药的滥用仍然会阻碍术后恢复。 引言:确定在化疗后腹膜后淋巴结清扫(PC-RPLND)患者中实施ERAS方案是否有助于减少术后疼痛和阿片类药物的使用。 方法:对具有PCRLPND适应症的连续癌症患者进行病例对照研究,这些患者接受常规术后管理(CPM)或ERAS方案。感兴趣的结果包括视觉模拟评分(VAS)-评估术后第3、7和30天的疼痛水平,以及吗啡当量剂量(MEDs)/术后天数。记录术中参数和术后并发症。采用参数检验和非参数检验进行统计分析。 结果:总共研究了100名阿片类药物初始PC-RPLND患者。CPM和ERAS组(分别为36和64名患者)具有相似的人口统计学和基线临床特征)。ERAS组患者的失血量(p=0.005)、输血率(p<0.001)和手术持续时间(p<0.001)显著降低。术后并发症在各组之间具有可比性。在CPM组中,恶心和肠道疾病在数量上更为常见,但在统计学上并不更为常见。ERAS患者平均住院时间较短(5.3±1.4 vs.7.4±1.6天,p<0.001),每日MED较低(4.73±2.63 vs.7.04±2.29,p<001),术后第7天VAS评分较低(3.89±1.07 vs.4.67±1.17,p=0.001)。术后第3天和第30天两组的术后疼痛相似。 结论:PC-RPLND术后系统实施ERAS方案可改善疼痛管理,优化患者康复,并防止阿片类止痛药的过量处方。
