背景:孤立性功能肾(SFK)是先天性肾尿路异常(CAKUT)的一个亚组。虽然这些患者的预后在过去被认为是良好的,但大量的研究表明,不同程度的肾损害与这种情况有关。血清肌酐测量仍然是评估肾功能最常用的指标,尽管其局限性已广为人知。 目的:本文对先天性SFK的病理生理、诊断、并发症、预后、新型泌尿生物标志物的作用、治疗和随访进行综述和更新。 结果:由于多种因素,先天性SFK的自然史仍是一个未解决的问题。虽然还没有在人类身上得到证实,但布伦纳的过度过滤假说是解释出生时只有一个功能肾脏的患者肾脏预后不良的最具体理论。对能够评估肾功能和预测肾脏预后的新型泌尿生物标志物的研究已经开始,但对这一特定人群的研究仍然很少。在研究最多的标志物中,胱抑素C、EGF和NGAL对这些患者的随访显示出潜在的有用性。治疗仍依赖于寻找肾损伤和一般的肾脏保护措施。 结论:为了更好地了解先天性SFK的自然病程和新型泌尿生物标志物在这一特定人群中的作用,需要进行更长的随访时间的进一步研究。因此,有可能改善这些患者的预后。
